Online Description

This is a 1 month contract for unlimited visits to "Conscious Singles" each week to be charged with a monthly auto pay.

Agreement Terms:

Charges will appear once on sale date, and again once per month, after sale date. No transfer policy, cancellation only upon written notice, see terms below.

Terms and Conditions:

a) I agree to purchase the 1 month contract at $120/month, as an automatic charge to my credit card.
b) I hereby certify that I am the holder of the credit card, or an authorized signer on the credit card provided.
c) I understand that I will be notified if my credit card payment fails to authorize for any reason, and that it is my responsibility to provide a new credit card within 5 days.
d) Cancellation must be given 15 days prior to the next scheduled auto payment. Cancellations given less than 15 days from upcoming auto payment date will not apply until the following auto payment.
e) I understand that Breath+Oneness reserves the right to amend or add to these conditions and adopt new conditions as it deems necessary for proper management of the business. Changes will be posted in the center if applicable.
f) Facilities or services may be unavailable due to mechanical breakdown, fire, act of God, condemnation, loss of lease, catastrophe or any other reason. Breath+Oneness shall not be liable for such occurrences.
g) Breath+Oneness is not responsible for the loss or theft of personal property. Members and clients are advised to not bring their valuables to the center.
h) I understand that this agreement is legally binding in terms and conditions, whether or not I use the Center and it’s services. The above conditions and future amendments will apply as long as I am a member of Breath+Oneness.
i) I understand that, if by reason of death or disability, I am not able to use the center, then my estate and myself will be relieved from the obligation of making payments for services other than those received prior to death or onset of disability. Any disability condition has to be such that I am physically not able to use the Center and such condition has to be verified by an affidavit of a physician.
